He experiences frequent hypertensive crises with blood pressure rising to 230/100 mmHg, headaches, dizziness, severe weakness, shortness of breath with minimal exertion, irregular heartbeats, and memory loss.

He walks with a cane and assistance.

Doctors diagnose stage III hypertension, CHF Class II, CKD C2, cardiac arrhythmias, and sequelae of hemorrhagic stroke with severe neurological deficits.

The patient is recommended to undergo a rehabilitation course with occupational therapy, physical therapy, massage, and observation by a neurologist to restore motor function, speech, and improve independence.
